PA and NJ Licensed Telehealth Physician

To support patient access in a growing primary care network, a full-time PA and NJ Licensed Telehealth Physician will provide remote telehealth services, focusing on patient outreach, annual screenings, and chronic disease management while collaborating with primary care teams. Key responsibilities Provide patient-centered care and ensure an excellent patient experience Conduct annual wellness visits and required screenings for patients Assist patients with chronic disease management and facilitate their reconnection to primary care providers Required qualifications MD or DO degree, or equivalent Board certified or eligible in Family Medicine or Internal Medicine 3+ years of primary care experience Licensure in both PA and NJ required, or ability to obtain by start date Commitment to health equity and care for medically and socially vulnerable populations
